Collaborative care, fully staffed.
Collaborative care for primary care practices, fully staffed. We provide the care manager, the psychiatric consultant and the registry, and you bill CoCM for patients you already see.
Why Triad
Primary care sees most depression and anxiety but rarely has time to manage it. CoCM codes pay for that work, yet practices need a care manager, a psychiatric consultant and a registry before they can bill.
Built for independent primary care and pediatric practices, FQHCs and direct primary care groups.

Questions
How do you implement the Collaborative Care Model?
We set up the registry, workflows and patient consent in a few weeks. Our licensed care manager then follows each patient, our psychiatric consultant reviews the caseload weekly, and each month you receive claim-ready CoCM codes with time documentation. Our guide to starting a Collaborative Care program walks through the team, consent and the first 90 days.
What does a behavioral health care manager do?
The care manager follows each enrolled patient between visits, tracks symptoms with the PHQ-9 and GAD-7, and provides brief interventions. The psychiatric consultant reviews their caseload each week and recommends treatment changes.
Should we staff collaborative care ourselves or with Triad?
If you need the care team, our Staffed plan provides the care manager, psychiatric consultant, registry and billing support. If you already have a care manager, our Software only plan gives you the registry with automatic time capture.
Which payers cover CoCM?
Medicare and many commercial plans do, and a growing number of state Medicaid programs.
Who bills?
Your practice bills under your treating provider. We supply documentation.
Do patients see a new provider?
Patients stay with their primary care provider; our care manager supports them between visits.
How fast can we start?
We set up the registry, workflows and patient consent in a few weeks, and then you can enroll your first patients.
What CoCM codes can my practice bill?
Each month you can receive claim-ready documentation for Collaborative Care codes 99492, 99493, 99494 and G2214. Our time thresholds guide shows how monthly care manager minutes map to each code.
How much does Triad cost?
The Staffed plan is 40% to 50% of CoCM collections and includes the care team, registry and billing support. Software only is $12 to $20 per enrolled patient per month for practices with their own care manager.
