Who writes our guides
The Triad team writes every guide on this site. Guides are general information for primary care practices that run or plan to run Collaborative Care (CoCM). They are not legal, tax, financial, billing or medical advice.
How we source guides
Billing and program details come from primary sources: CMS publications such as the Behavioral Health Integration Services booklet (MLN909432), Medicare manuals and Physician Fee Schedule materials, and the public CPT and HCPCS code descriptions. Each guide ends with a Sources and check date section that links the sources it relies on.
Numbers in a guide come from the sources it lists. Sample figures are labeled as examples.
How dates work
- Published is the date a guide went live.
- Updated appears when we change a guide after it goes live.
- The check date in the sources section is the date we last checked the guide against its sources.
Code rules change every year. Confirm current code text, time rules and payer policy with your Medicare Administrative Contractor or Medicaid program before you bill.
Expert review
When a credentialed billing, coding or clinical professional reviews a guide, we name the reviewer and the review date on that guide.
How we update guides
When CMS or CPT changes a rule a guide covers, we update the guide, change its Updated date and refresh its sources and check date. We check guides that are scheduled to publish against current sources before they go live.
How to report an error
If you spot a mistake, use the contact form on our home page. Include the page address and what looks wrong. We review every report and correct confirmed errors.
Our business
Triad sells Collaborative Care staffing and registry software. Guides describe our service only in the How Triad helps section at the end, kept apart from the guidance.
